About Chronic Neuropathy Pain

Neuropathic pain is caused by a lesion or disease of the somatosensory system, including peripheral fibres (Aβ, Aδ and C fibres) and central neurons, and affects 7–10% of the general population. Multiple causes of neuropathic pain have been described and its incidence is likely to increase owing to the ageing global population, increased incidence of diabetes mellitus and improved survival from cancer after chemotherapy. Neuropathic pain may result from disorders.

Chronic Neuropathy Pain Epidemiology

Neuropathic pain is characterized by a “pain arising as a direct consequence of a lesion or disease of the somatosensory system”. The existing literature has primarily investigated the epidemiology of neuropathic pain within the context of specific diseases, such as cancer, diabetic peripheral neuropathy (DPN), and chronic low-back pain (CLBP). The study by Bouhassira et al. (2019), have shown that the prevalence of neuropathic pain in the general population may be as high as 7–8%, accounting for 20–25% of individuals with chronic pain.

Important facts of Chronic Neuropathy Pain Epidemiology Report

  • Chronic neuropathic pain is more frequent in women (8% vs. 5.7% in men) and in patients >50 years of age (8.9% vs. 5.6% in those 12,000 patients with chronic pain with both nociceptive and neuropathic pain types, referred to pain specialists in Germany, revealed that 40% of all patients experience at least some characteristics of neuropathic pain.
  • A study of German population has shown that in general 6.5% of respondents have reported pain with chronic neuropathic pain characteristics, and have reported higher intensity and more interference of pain in their daily activities as compared to individuals with chronic non-neuropathic pain.
  • “Neuromodulation Management of Chronic Neuropathic Pain in the Central Nervous System,” chronic neuropathic pain is a widespread condition that affects 7–8% of the total population.

Chronic Neuropathy Pain Diagnosis

Neuropathic pain may result from disorders of the peripheral nervous system or the central nervous system (brain and spinal cord). Thus, neuropathic pain may be divided into peripheral neuropathic pain, central neuropathic pain, or mixed (peripheral and central) neuropathic pain. Examples of the peripheral neuropathic pain include diabetic peripheral neuropathy (DPN), postherpetic neuralgia (PHN), antineoplastic therapy or HIV-induced sensory neuropathy, tumor infiltration neuropathy, phantom limb pain, postmastectomy pain, complex regional pain syndromes (reflex sympathetic dystrophy), and trigeminal neuralgia.

Chronic Neuropathy Pain Treatment

The treatment initially focuses on resolving the underlying cause. Medications used to treat neuropathic pain include over-the-counter analgesics, anticonvulsants, tricyclic antidepressants (TCAs), topical anesthetic agents, nonsteroidal anti-inflammatory drugs (NSAIDs), antiarrhythmics, narcotic analgesics, and opioids.
